Sometimes an individual may need to request a replacement (also known as a duplicate) of their current driver license or ID card because the original was lost, stolen, destroyed, etc.
NOTE: Individuals who need to request a replacement card to change their address should visit Change Information on Your Driver License or ID Card.
To apply for a replacement driver license or ID card, individuals must:
Individuals will also be asked to confirm personal information, such as date-of-birth or Social Security Number. If the Social Security Number is not already part of the driver record, then the individual must provide documents to verify his/her Social Security Number. (Documents verifying a Social Security Number are required for a driver license but voluntary for an ID card.)
Individuals must file a police report if their driver license or ID card was stolen and used by someone else.
